On the Immunization Reconciliation Form, what fields are used to match a patient between CPS and the Registry?

The fields that the registry uses to match a patient differ between registries, however, most registries match on  a minimum of; first name, last name, date of birth and gender. Possible additional fields include address, phone number and mothers maiden name.

The Immunization form uses the Registration information for the patient that the form is called from. The following information is then sent to the state. The State Immunization Registry then does a match on this information.

  • mrn
  • lastName
  • firstName
  • middleName
  • ssn
  • dob
  • phone
  • address
  • city
  • state
  • zip
  • gender
